In line with the NHS Patient Safety Program, Dr Hardman & Partners have agreed that the clinical risk associated with accepting repeat prescription requests over the telephone needs to be limited.
Why Are We Changing?
- Our new systems will be safer for patients.
- Some requests for medication over the phone are not clear and it can take considerable time and effort to clarify what is required.
- Written requests avoid potential confusion and medication errors.
- In addition to improving patient safety it will reduce the number of telephone calls to the practice which will improve telephone access for those in need of emergency help, advice or making an appointment.
Repeat Prescriptions - You can order a prescription for medication by:
Post: enclosing the order slip from your previous prescription (marking the medication you require) together with a stamped, addressed envelope, unless you want to collect the prescription from the surgery or a pharmacy. If so please make this clear.
In person: Again, using the order slip from your previous prescription and marking the medication you require.
On-line: Using our on-line prescription request form – Ask at Reception for details on how to register for online service.
At least 48 hours notice (two working days) are needed to have your prescription ready for collection. Many patients use this service so please help us by ordering as early as possible.
Urgent & late requests put considerable strain on the Practice staff and the service we provide our patients.
Please leave 72hrs when requesting collection from a chemist and 5 working days when the chemist needs to deliver.
Named Accountable GP
The practice is required by the Government under the terms of the latest GP contract to allocate all patients a named accountable GP. Individual patients will be informed of their named accountable GP at the first appropriate interaction with the practice.
¨All current and new patients registered with this Practice will be allocated a named GP.
¨ If you would like to express a preference as to which GP you are assigned the Practice will make reasonable efforts to accommodate this request.
¨ Your accountable GP is responsible for your overall care at the Practice, however you are welcome to see any GP within the Practice.
¨If you would like to know the name of your accountable GP please ask at the Reception desk.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative Doctor.

We believe the Medical Centre is easily accessible for those with disabilities with all patient services on the ground floor, corridors wide enough for manual wheelchairs and the provision of a disabled toilet at the front of the building accessed using a Radar key.
